In 2009 an independent examination into the tribunal reported there was no evidence to support Justice Keane's finding that the fire was started deliberately near the ballroom of the nightclub. In a statement reported by the BBC, he said: “I researched Hillsborough from 1989, publishing reports, articles and the first edition of Hillsborough: The Truth in 1990. Until 2009, and despite compelling evidence, successive governments declined to pursue a thorough independent review of the context, consequences and aftermath of the disaster. The inquest is hearing from families, witnesses, and experts and is expected to take about six months. A tribunal held in the year following the fire, chaired by Justice Ronan Keane, was labelled flawed and its conclusion was contested by the victims' families.

The Shipman Inquiry - Reports into serial killer and general practitioner Harold Shipman. This inquiry held that while Shipman was convicted of murdering 15 of his patients the true figure was at least 215 with the possibility that he may have killed as many as 260 patients. This changed as a direct result of the families’ and survivors’ brave, persistent campaign. It led to the Hillsborough Independent Panel, its groundbreaking findings, new inquests and their crucially significant verdicts.” Scraton's book Hillsborough: The Truth is now widely accepted as a definitive account of the disaster and its aftermath. [7] The book focuses on the inadequacies of the police investigations, official inquiries and inquests, revealing the extent of the systemic review and alteration of South Yorkshire Police statements. It also details the treatment of the bereaved and survivors in the immediate aftermath of the disaster, and the "inhumanity" of the body identification process. At the time of its first edition (1999), the book was described by Liverpool playwright Jimmy McGovern as a "brilliant achievement" [4] and The Independent stated that it told "a scarcely believable story of incompetence and mendacity". [4] Two new editions were published, in 2000 (following the private prosecution of two senior officers), and 2009 (detailing the previous decade's campaign for justice). Reflecting on the work of the Hillsborough Independent Panel, a fourth edition was about to be published in 2014 when, at the new inquests into the deaths of the (at the time) 96, the Coroner imposed stringent 'contempt of court' regulations on publications and broadcasting. Following the conclusion of the inquests in mid-June 2016 a revised fourth edition was published, including three new chapters on the work of the Panel, its political impact and the new inquests.

Families of the victims of the fire gathered on Tuesday morning at the Garden of Remembrance in Dublin and walked the short distance to the Rotunda Hospital campus where the inquest is being held.Prof Phil Scraton, who led the Hillsborough Independent Panel's research team, says the fresh inquest into the Stardust fire in Dublin is of "paramount importance" to the families of victims, survivors and “the people of Ireland”. Scraton’s book, Hillsborough: The Truth, is regarded as a definitive account of the disaster and he was given the freedom of the city of Liverpool for his work. a b c d e Scraton, Phil (2009). Bartlett, David. "Primary author of Hillsborough Independent Panel report Prof Phil Scraton to receive special award from Queen's University Belfast". Liverpool Echo. Responsibility for the collapse in authority, management and safety, lies with those organising and administering the event. ” After a long campaign by the Stardust families, in 2019 then attorney general Seamus Woulfe directed that new inquests take place. The same investigating officers had served all three inquiries into Hillsborough thus compromising their independence, the families did not receive legal aid and had limited disclosure of evidence. The report revealed a range of other systemic failings, most significant being the Coroner's imposition of a 3.15pm cut-off on evidence - a time when some who died were still alive. Effectively this inhibited examination of evidence relating to the effectiveness of the rescue and treatment of the fans. It presented 87 recommendations for handling the aftermath and legal processes following disasters. [ citation needed] Stuart-Smith Scrutiny [ edit ]

An inquest jury concluded in April that the 96 who died in the crush at Hillsborough stadium on 15 April 1989 were unlawfully killed. This has been a long-running sore. We are talking about over 40 years. We are talking about the number of deaths - 48 people died - so many people damaged for life," Prof Scraton told BBC Radio Ulster's Evening Extra programme. Both my parents, John and Christine Keegan, put an awful lot of effort into this and it consumed their lives to get truth and justice put on public record."
